Here are 15 powerful assessment questions to establish where someone currently stands in their physical health journey (fitness, nutrition, and overall wellbeing):


Physical Fitness

  1. How would you rate your current physical fitness level (1–10)?

  2. How many days per week do you engage in structured physical activity or exercise?

  3. Do you include strength training, cardiovascular exercise, and flexibility work in your routine?

  4. How easily do you get tired during daily activities (e.g., walking stairs, carrying groceries)?

  5. Do you experience any regular pain, stiffness, or mobility limitations?


Nutrition & Eating Habits

  1. How would you describe your current eating habits?

  2. How many servings of fruits and vegetables do you consume daily?

  3. How often do you eat processed or fast foods?

  4. How much water do you drink per day?

  5. Do you eat mindfully, or do you often eat while stressed, distracted, or rushed?


Recovery & Body Maintenance

  1. How many hours of sleep do you get on average per night?

  2. Do you wake up feeling rested and energized?

  3. How do you manage stress, and does it affect your physical health?

  4. When was your last medical check-up or health screening?

  5. Are there any current medical conditions, medications, or ongoing health concerns affecting you?
